- Pre-admission Health History
- Physician’s Report
- Notification of Parent’s Rights
- Notification of Personal Rights
- Field Trip Waiver
- Consent for Medical Treatment – TWO ORIGINAL COPIES REQUIRED: one for school files, the other accompanies students on all field trips
- Photo Release Form
- Support Job list-2025-26
- Orientation Announcement 2025- to be posted August 2025

Level 1 / 1.5 Parent (working in classroom) Requirements:
- TB Test
- Vaccine proof of DTaP/ TDaP & MMR
Level 1/ 1.5 and 2 Parent Requirements:
- Copy of Drivers Licenses/ ID
- Copy of Current Auto Insurance
